Support Coordination is a service that helps you work out the best way to organise your NDIS supports. Support Coordination takes the stress out of managing your supports, gives you peace of mind, and ensures you get the most out of your NDIS plan.

There are 3 different levels of support coordination available through the NDIS.

  • Level 1: Support connection
  • Level 2: Coordination of supports
  • Level 3: Specialist support coordination

What does a Support Coordinator do?

Support Coordinators work with you to find out what your NDIS goals are, and how to achieve them. Your Support Coordinator can help you manage and maximise your supports in a number of different ways including understanding your plan, selecting supports, and connecting with relevant services.

What are the benefits of having a Support Coordinator?

  • Less stress and more time to focus on your goals.
  • Helps you to build independence, make more informed decisions, and exercise choice and control.
  • Offers assistance to find, negotiate and manage service agreements with a variety of service providers (both short and long term).
  • Provides ongoing support to show you and your family/carers how to manage your NDIS plan and get the most out of your funding.
  • Links you with community, mainstream, and other government services.
  • Helps you to prepare for your NDIS plan reviews.

How do I know if I have funding for a Support Coordinator?

If you are eligible for Support Coordination there will be funds allocated in your Capacity Building Support budget.

Most people will only receive funding for Support Coordination when they receive their first NDIS plan, or when they need some extra support, for example, if they move into supported accommodation, or are experiencing long-term health challenges.

The amount of funding you receive is based on what the NDIS has deemed is “reasonable and necessary” to pursue your goals.

If you don’t have funding for Support Coordination included in your NDIS budget, or need to change your current level of funding, you can talk to your Local Area Coordinator (LAC). They will be able to help you determine if it’s possible to receive funding for Support Coordination. It’s a good idea to bring as much evidence as possible (reports, assessments, and quotes) with you when you speak with your LAC.

There are many reasons why people may request funding for Support Coordination services including:

  • communication challenges, or high/complex care needs
  • changing health requirements
  • family situation or lack of support network
  • cultural or language barriers
  • new to the NDIS
  • need assistance with making informed decisions about services, providers, and budget

We work with providers, and community and mainstream organisations across Queensland to provide customised care and support, and maximise your NDIS funds.

When you start with HELP our Support Coordinators will sit down with you and your parent or carer to form an Action Plan.

Your Action Plan

An Action Plan is a document that records agreed strategies and steps for reaching your goals. Your Action Plan is tailor-made to address your specific needs and is also used to record your progress. Our Support Coordinators will check your plan regularly to make sure everything is on track, and that you are receiving the best support to help you reach your NDIS goals.
